From 1st April 2015 we are required under our NHS contract to allocate a named accountable GP who will be responsible for your overall care to all patients.  This has been done for all existing patients.  All newly registered patients will be given a named accountable GP on registration.

In order to facilitate this we have allocated patients to a named GP on a surname basis as follows:

Patients with a surname beginning with A-G: Dr Young Patients with a surname beginning H-R: Dr George Patients with a surname beginning S-Z: Dr Rookwood

If you are aged 75 and over and registered before 1st April 2015 you will have already been allocated a named accountable GP by letter – this named GP will remain the same and will not change under the surname arrangements above.

Please be aware that you may still see any GP of your choice at the surgery and do not have to see your named accountable GP, nor does this mean that you will only be offered apppointments with your named GP.

You do not need to take any further action, but if you have any questions, or wish to discuss this further with us, please contact us on 0191 2537892 or at sa87022@nhs.net

All of our patients have now been informed of their named GP as of 31st March 2016.
