Register with us as a New Patient

The practice welcomes new patients who live within its practice boundary.  Our boundary covers a wide area including Shiremoor, Backworth, Holystone, West Allotment, Earsdon, West Monkseaton, Murton, Holywell and New build estate off Rake Lane (Robinson Fields/Murton Gap) If you are unclear if your address is included please ask our reception staff to check for you. You can also check our Area Map online, click on the map, click on the magnifying glass in the top right corner and enter your postcode, if your postcode is within our practice boundary this will fall within the shaded area.

The Practice has a historic outer boundary, covering parts of Seghill and Seaton Delaval, we are only able to accept registrations from dependants of existing registered patients from these areas. Please enquire with reception to confirm your eligibility to register if you live in these areas.

To enable us to register you with the practice you will need to complete some forms and provide proof of address and identification. You can complete these forms online using our online service, or pop into the practice and ask our receptionist for details. We are unable to book any appointments until you are registered.

Registrations take a minimum of 7 working days to process.

There are a number of reasons why you may not be able to register with your chosen GP, for example, you may live too far away. If this is the case simply choose another GP in your local area.

Please note that your medical records may take several weeks to arrive at the Practice.

Register Online

New patients just need to fill in an online form to get started. You do not need proof of address or immigration status, ID or an NHS Number. The service matches you to the Patient Demographics System (PDS). Where the service cannot match you – ie; you have never been registered in the UK before, the practice may ask for forms of ID for verification processes.

This service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

If you would rather register with a paper form, please call into the surgery and ask reception for the two forms along with copies of two forms of proof of identity.

Summary Care Records

Important information regarding your NHS medical records

The NHS in England is introducing the Summary Care Record, which will be used in emergency care situations.

The record will contain information about any medicines you are taking, allergies you suffer from, and any bad reactions to medicines you have had to ensure those caring for you have enough information to treat you safely.  This information will be available to authorised health care staff providing your care anywhere in England.
We recommend that you read the information available from the NHS by visiting NHS Digital Summary Care Records (SCR) or by telephoning the helpline on 0300 123 3020.
Your decision is required when registering with Northumberland Park Medical Group.
